FEE AGREEMENT

By submitting your fee of $375.00, you agree that I will provide you a Will, Living Will, and Power of Attorney based on the information that you provide. You understand that the documents I provide are based solely on the information that you have given me.  If your information is inaccurate or incomplete, new documents will not be provided without an additional fee.  You understand that my only obligation to you is to provide the above referenced documents.  I will not provide any further legal services.  The fee paid is in payment for the drafting of said documents and no other legal services.  You understand that once the documents are received by you, our attorney/client relationship is severed and no further work will be performed by me on your behalf. You further understand that should your circumstances change, you will be required to re-hire me or another attorney in order to change or re-draft your personalized estate documents.
